I UNDERSTAND THAT IF GIVEN AN ASSIGNMENT, MY PAY STARTS WHEN I REPORT TO EXACT STAFF'S CUSTOMER AND I WILL BE PAID FOR HOURS ACTUALLY WORKED. I UNDERSTAND THAT ON SOME JOBS, EXACT STAFF'S CUSTOMERS MAY OFFER ME REGULAR FULL TIME EMPLOYMENT. I ALSO UNDERSTAND THAT EXACT STAFF HAS AGREEMENTS WITH ITS CUSTOMERS WHICH GENERALLY PROVIDE THAT EXACT STAFF EMPLOYEES MUST BE PLACED WITH THE CUSTOMER FOR A MINIMUM OF 520 CONSECUTIVE HOURS BEFORE THE CUSTOMER CAN OFFER REGULAR FULL TIME EMPLOYMENT TO THE EXACT STAFF EMPLOYEE. SHOULD I BE OFFERED FULL TIME EMPLOYMENT BY ANY EXACT STAFF CUSTOMER, I AGREE TO IMMEDIATELY NOTIFY EXACT STAFF.

I UNDERSTAND THAT AS A CONDITION OF MY EMPLOYMENT, I MAY BE REQUIRED, PRIOR TO AND/OR DURING EMPLOYMENT, TO TAKE AND PASS DRUG AND ALCOHOL TESTS. BY SIGNING THIS APPLICATION, I CONSENT TO THE TESTS.

CLASS OR COLLECTIVE ACTIONS ARE NOT PERMITTED BY THIS ARBITRATION AGREEMENT.
